Open the Personal Evaluation Form
  Click on the Performance Evaluation link to download a copy of the form:
    Evaluation for Monthly/Salaried Employees
    Statement of Policy for Performance Evaluations
    New Evaluation for Hourly/Classified Employees
   
 
Filling out the Performance Evaluation Form
 
Evaluation Period: April 1 through March 31.
 
 
 
Any employee hired before 1 April 2008, should have a
full performance evaluation completed and turned in to
the Payroll Office by 31 May.
 

If you have an employee who transferred to your
area during the evaluation period, the evaluation should be a
joint effort between the supervisors.
 

If you have an employee who is out on leave during
the evaluation period, please evaluate that employee promptly
upon their return.  Submit the completed evaluation to the
Personnel & Payroll Office as soon as possible.
